A MAN WITH RECURRENT PRESYNCOPE CONDITIONS

A forty-five-year-old patient with hypertrophic cardiomyopathy (HCMP) with repeated presyncope conditions. A standard 24-hour ECG Holter was used and sinus rhythm was present during the entire monitoring period with quite rare ventricular extrasystoles, 2× in the triplet.

We recommended to the patient the use of long-term ECG monitoring by means of an episodic recorder with automatic detection of rhythm disturbances until such time, when a presyncope state occurs.

On the 6th day of monitoring, we captured wide complex tachycardia (NSVT) amounting to 22 complexes, when the patient experienced rapid, irregular heart beats at the frequency of about 5 per second. We contacted the patient’s attending cardiologist, who then sent the patient to the cardiologic centre. Based on the results of the examination, implantation of cardioverter defibrillator (ICD) was recommended in order to reduce risks of sudden death.
