A PATIENT WITH UNEXPLAINED SYNCOPE

A 65-year-old patient suffered from subjective difficulties with irregular heartbeat leading to recurrent syncopes. The cause of the difficulties was not clarified either during outpatient examinations or during the 24-hour long Holter ECG examination.

Based on recommendations from her cardiologist, the patient ordered an episodic monitoring machine from us.

During monitoring, incoming ECG strips showed a sinus rhythm with more frequent atrial ectopy occurring in isolation (Fig. 1) and in volleys or in jumps of irregular supraventricular tachycardia. Already on the third day of monitoring, jumps of atrial tachycardia (Fig. 2) progressed into repetitive paroxysms of atrial fibrillation, with termination into sinus rhythm with post-tachycardia pauses until 11.9 seconds at night (Fig. 3). The cardiologic centre, which had sent the patient, was informed about this recording, and the patient was subsequently hospitalized and provided with medical treatment in the same cardiologic centre.
